Request account deletion

Use this form if you cannot access enodoAlly to start an account-deletion request.

If you can sign in, use Profile → Delete account. That immediately ends access and begins the existing seven-day pending-deletion period.

We verify control of the account email before processing an external request. A clinic or clinician may control care records and may need to handle records it controls separately.
